Once you pay for your analysis/reading electronically on this site and send Scott the requested data/forms, Scott will email you back or call you to schedule. He will then call you at the appointment time if you live within the continental U.S. If you live outside of this area, you'll have to call Scott at the agreed upon appointment time. If you don’t hear back from Scott via email or phone within 72 hrs after placing your order electronically, assume there was a technical problem that prohibited him from receiving your order by email. For non-handwriting analyses (numerology/astrology), it’s sufficient to simply email the requested content of the S. A. Petullo Order & Contact Information Form (Corporate or Personal, see above) here instead of faxing the information.

Below is a listing of some of the information requested on the S. A. Petullo Order & Contact Information Form and is necessary for Numerology/Astrology Analyses (Corporate or Personal).

*Current name and full, legal birth name (exactly like it is on the original birth certificate).

*Month, day, year, exact time, and location of birth (state “approximate” if you don’t know exact time of birth).

*All of the above information for other people involved, if appropriate (compatibility, group analyses, etc.).

*Date and approximate time you first met the person in question; or date and approximate time you first had the idea, started the project, incorporated the business, signed the contract, etc.

*Please state what you want in a consultation, the key issues to be addressed, and your related concerns: Love life? Incorporation? Product launch? New hire? Career? Money? Family? Event timing? Forecast?

*Indicate several dates and times when you are available to have the consultation.

Handwriting Samples:

Please see "how to prepare a handwriting sample" above. In general, the sample should be about one page or more in length, and a ballpoint pen or similar writing instrument should be used (not pencil). The sample should be written in cursive (at least one paragraph of cursive if printing is habitually used) using the S. A. Petullo Handwriting Form (see above), or with regular 8½ x 11 unlined paper. The instructions on the S. A. Petullo Handwriting Form about what to write should be followed. Something else spontaneous may be written, such as plans for next week, or what was done yesterday, etc. The sample must be signed and dated.
